Registration Forms

TO REGISTER FOR an OWU Intramural event, please click on the appropriate link below and print off the registration form. Complete all parts of the form (make sure to get signatures for all parties involved in a team sport) and return the form prior to the due date for your sport. Forms may be turned in to Office 116 in Edwards Gym or left in the appropriate envelope on the bulletin board outside Office 114 in Edwards Gym.
